Contractor Qualification Form – Document Extract
Our Contractor Pre-Qualification Form provides a list of safety-related questions to help you determine the qualifications of a potential contractor. Questions include:
- Do you have a Safety Policy? If yes, please attach
- Do you have the appropriate qualification to undertake this task? If yes, attach copy of relevant qualifications
- Do you have the appropriate Safe Work Method Statements for the tasks undertaken? If yes, attach copy of SWMS for each activity
- Has the company ever been convicted of an occupations health and safety offence? If yes, provide details
- Does your company have the appropriate Workers Compensation, Public Liability & Professional Indemnity Insurances? Attach copies
- Does your company have the relevant permits? Attach copies
- Do all your employees have the appropriate qualifications? Attach all employee details who will be engaged in the stated work
- Are all employees inducted into the safety precautions of the site they are working at?
